Referring Patients to Rehab: A Guide for Social Workers & Community Health Workers
Social workers are trained to assess, treat, and advocate for clients with substance use disorder. Many patients enter addiction treatment after being referred to a rehab center by a social or community health worker.
Referring clients to a treatment facility can be invaluable in helping someone in dire need turn their life around. This page will guide social workers through the referral and admissions process at American Addiction Centers (AAC) facilities.
How Does The Referral Process Work?
When working with social and community workers to connect patients with the help they need, AAC provides the highest standard of excellence throughout the entire process. Our representatives will manage all logistics, including:
- Verification of the patient’s benefits.
- Patient evaluation.
- Obtaining treatment authorization from the patient’s insurer.
- Handling all communication with the insurance company.
- Connecting the patient with the treatment facility that meets their clinical needs within their financial limitations.

Types of Addiction Treatment We Offer
There are several different levels of addiction treatment. The appropriate type of treatment will vary based on the patient’s needs, including the severity of their addiction, their living environment, social support, and more. AAC facilities may offer:
- Medical detox. Our inpatient medical detox programs can safely stabilize patients through acute withdrawal before they move on to the next phases in treatment.
- Residential treatment. With residential or inpatient care, patients live at the facility, benefitting from around-the-clock support and a structured routine.
- Partial hospitalization programs (PHPs). A step down from residential treatment, PHPs are the most intensive form of outpatient care. Patients in our partial hospitalization programs typically spend 5 days a week in treatment for 6-hour session blocks.
- Intensive outpatient programs (IOPs). Our IOPs generally require patients to visit the facility for treatment 3-5 days a week.
- Standard outpatient treatment. Standard outpatient care affords the most flexibility in scheduling and is appropriate for patients with work or school obligations, lower risk of relapse, and robust social support.
- Sober living. Following treatment or while enrolled in an outpatient program, many of our patients benefit from the supportive environment of peers and mentors that a sober living facility provides.
Experienced staff at our facilities can help patients with a variety of addictions as well as those needing treatment for co-occurring mental disorders like depression, anxiety, posttraumatic stress disorder (PTSD), obsessive-compulsive disorder (OCD), and others.
Our Riverview, Florida facility also offers a residential mental health program for people without substance use issues.
Specialized Care for Veterans
AAC facilities are also equipped to meet the needs of Veterans, with a Veterans team made up of com-psych certified Veteran liaisons and experienced, compassionate leadership.
Our facilities provide specialized care for Veterans through programs like Salute to Recovery. This program emphasizes:
- Strength-based approaches: We honor each Veteran’s individuality and resilience, focusing on their strengths, fostering positive self-image, reshaping thought patterns, and offering sincere, non-judgmental support.
- A holistic approach to readiness: We prepare Veterans mentally, physically, and spiritually, while developing essential skills such as coping with triggers and practicing grounding techniques.
- Empowering change: Using the Mission Focused Mindset, Veterans are encouraged to tap into their inner strength and take charge of their personal transformation.
